To Refer a Patient to a Specialist: Please use our online Patient Referral Form for Specialty Services to refer a patient to one of the following services: Cardiology, Dentistry & Oral Surgery, Dermatology, NeurologyInternal Medicine, or Surgery. If you would prefer to complete a paper form, please complete our Specialty Referral Form. To refer a patient for physical rehabilitation through The Sport & Strength Department, please use our Rehab Referral Form. Paper forms can be faxed to 651-501-3763 (or, in the case of Dentistry & Oral Surgery, 651-291-1337) or emailed to aercminnesota@gmail.com.
